Current BSA Health and Medical Records are required for all Boy Scout activities. Parts A & B do not require a doctor’s signature. Part C does.
Thinking Ahead – On your next visit to the doctor’s office take the form with you.
Note that Parts A, B and C are required in order to participate in summer camp, so it’s essential that these completed forms be turned in to the adult support for that particular event. Also, any scout that will be attending any of the BSA High Adventure bases (including Sea Base) must also have their physician complete Part D.
